Health Record is a comprehensive personal electronic health record (EHR) application built for individuals and families who want a real, structured, standards-based record of their own health — not a fragmented mix of PDFs from different providers and screenshots of lab results.
Philosophy
Your medical history belongs to you, not to whichever provider's portal happens to hold a piece of it. Health Record is hosted for you, so there's no server to set up — you sign up and start, and nothing is sent to a third party unless you explicitly connect an integration or generate an export. There's no ad-supported tier, no data-sharing business model, and no vendor lock-in — your data is portable in open formats from day one. If you'd rather run it on your own infrastructure instead, the same backend supports self-hosting — get in touch and we can talk through it.
Standards & Interoperability
Health Record isn't a free-text notes app wearing a medical skin. Conditions are coded in ICD-10 and SNOMED CT, lab results and vitals carry LOINC codes, and the export pipeline speaks FHIR R4, FHIR IPS, HL7 v2, CDA, and SMART Health Cards alongside plain formats like PDF, CSV, and Markdown — thirteen formats in total. That means your record can actually move: into another EHR, into a wallet app as a QR-verifiable SMART Health Card, or just into a readable PDF for a visit.

Works With Your Existing Devices
Connect these providers from Settings to sync data automatically instead of entering it by hand — see the User Manual's Integrations section for details on how syncing works.
