What belongs here
Not everything fits neatly into a structured field. Discharge summaries, imaging reports, referral letters, insurance paperwork, and scanned paper records all belong in Documents — a place to keep the original file alongside the rest of your structured record, rather than losing it in an email attachment or a folder on a computer somewhere.
Uploading a document
From the Documents section, Add lets you upload a file, give it a title and date, and attach a short note describing what it is. Once uploaded, it sits alongside your conditions, labs, and vitals as part of the same record — not a separate system you have to remember to check.
Why keep the original file
Structured data (a condition, a lab value) captures the meaning of clinical information, but the original document is often still worth keeping — a radiologist's full imaging report has detail a single structured Observation can't fully capture, and some documents (insurance forms, signed consent) matter specifically because they're the original file, not a summary of it.
Storage
Uploaded documents are stored alongside everything else in your record — the same patient-owned-data principle covered in the About page applies here: nothing is uploaded to a third-party document service.
